
Context:
The Union Health Ministry has introduced a structured national framework for screening, diagnosis, treatment, and long-term management of diabetes in children. It aims to ensure universal screening and free, continuous care under the public health system.
- Diabetes Mellitus in children is a chronic non-communicable disease, primarily Type 1 diabetes, caused by inadequate insulin production or autoimmune destruction of pancreatic beta cells.
- Early detection is critical as uncontrolled diabetes leads to hyperglycaemia, causing long-term damage to nerves, blood vessels, eyes, and kidneys.
- The framework promotes universal screening for all children from birth to 18 years, ensuring early identification of suspected cases.
- It integrates childhood diabetes care into the public health system, linking primary, secondary, and tertiary healthcare levels.
- Provides free-of-cost comprehensive care, including insulin therapy, glucometers, test strips, regular monitoring, and follow-up services.
- Emphasises district-level diagnosis and referral mechanisms, ensuring accessibility and timely treatment.
- Introduces the “4Ts” awareness framework—Toilet (frequent urination), Thirsty, Tired, and Thinner—for early symptom recognition.
- Focuses on family and caregiver empowerment, including training on insulin administration, glucose monitoring, and emergency response.
- Aligns with broader goals of reducing out-of-pocket expenditure (OOPE) and strengthening management of non-communicable diseases (NCDs) in India.
