Why in News?

Following suspected deaths in Shillong, the Meghalaya Government has activated the Integrated Disease Surveillance Programme (IDSP) to contain Neisseria meningitidis.

Description:

The Pathogen: Neisseria meningitidis

  • Nature: A Gram-negative bacterium; humans are the only natural reservoir.
  • Transmission: Direct contact via respiratory droplets or oral secretions (coughing, kissing, sharing utensils).
  • Incubation: Usually 3–4 days (range 2–10 days).
  • Key Risk: High fatality rate if untreated; survivors may face long-term neurological damage or limb loss.

Clinical Presentation:

Meningococcal disease primarily manifests in two ways:

  1. Meningitis: Inflammation of the brain/spinal cord membranes. Symptoms include neck stiffness, sudden high fever, severe headache, photophobia, and confusion.
  2. Meningococcemia (Septicaemia): Bloodstream infection. Symptoms include Fatigue, vomiting, cold hands/feet, and a characteristic non-blanching petechial rash (purple spots).

Public Health Response:

  • Surveillance: Active case searching and contact tracing by the District Surveillance Unit.
  • Chemoprophylaxis: Administration of antibiotics (usually Rifampicin or Ciprofloxacin) to close contacts to “clear” asymptomatic carriage.
  • Lab Review: Confirmation via Cerebrospinal Fluid (CSF) analysis or blood cultures.
  • Protocol: Isolation of suspected cases and strict adherence to respiratory hygiene in crowded settings (e.g., hostels/training centers).
